Grooming & Boarding Consent Form

    ​All boarders must be checked in/out Monday- Friday 8:00 a.m. – 5:00 p.m., or
    Saturday 8:00 a.m. – 2:00 p.m. There is no Sunday drop off or releases for
    boarders. Vaccinations are required for all pets entering our hospital for surgery,
    grooming and day boarding.

    ​Required vaccines are:
    DOGS:
    • Distemper/Parvo (DAPPV) 
    • Rabies 
    • Bordetella 

    CATS:
    • Distemper (FEL-O-Vax FIV) 
    • Rabies 

    Behavior Concerns

    If you are boarding more than one dog, you have the option to keep them in
    the same kennel. Understand that in different environments some dogs can
    react differently than they may at home. If we see any signs that they need to
    be separated we will do so, but we are not responsible for any accidents that
    may arise from them staying together.
    Complimentary Bath
    ​
    Any dog boarding for 5 days or more will receive a
    complimentary bath. If we see that your dog has gotten dirty, and needs a bath and has
    not been here the full 5 days, we will give that dog a bath but you will be responsible for
    paying for that service.

    Treatment While in our Care
    All internal and/or external parasites identified upon check in, or during the
    course of boarding will be treated at owner’s expense.

    If your pet becomes ill or if the state of your pet’s health otherwise requires
    medical attention, the DVM at his/her discretion, will administer treatment
    deemed necessary for the health and safety of your pet.

    I hereby give permission for The Animal Hospital to administer emergency
    treatment for my pet. I further agree to take responsibility for treatment and
    procedures received by my pet while boarding in the facility and understand
    that full payment is due upon discharge.

    Pick up Requirements
    Please pick up on the date that you have scheduled, we have limited space and
    have those spaces booked for other clients when one leaves.

    Any animal not picked up, without contact from the owner or agent, after 10 days
    from the agreed upon pick up date will be deemed abandoned. Then the animal
    will be turned over to the local shelter.
